New Patient Information
Welcome to Simard Foot & Ankle Clinic! Before your initial consultation, please take some time to review the information below. At your initial consultation, our Chiropodist will perform a full assessment of your feet and discuss a treatment plan with you. Your treatment can then begin!
Things to know before your appointment…
- Bring a list of all of your current medications.
- Bring your current footwear and orthotics.
- Referral from your physician/specialist if you have one, however you do not need one to become a patient.
- Please remove your nail polish so your nail health can be examined.
- We ask that you give at least one full business day's notice for all appointment cancellations.
Payment options…
- We accept cash, debit card, Visa, and MasterCard, American Express and eTransfer.
- Payment is due on day of service rendered.
- An itemized invoice will be issued for you to submit to your insurance company for reimbursement.
- Our fees are within the recommended fee guide as set by the Ontario Society of Chiropodists and Canadian Federation of Podiatric Medicine.
- Chiropody services are not covered by OHIP so all visit fees are the responsibility of the patient.
- Our services are typically covered by most third party insurance companies through workplace health benefit plans. Knowledge of insurance coverage details is the responsibility of the patient.
- Direct billing is available for the following:
- Greenshield for appointments.
- WSIB, if injury is related to feet.
- Veteran's Affairs for footcare and orthotics.
- Non-Insured Health Benefits (NIHB) for First Nations and Inuit for Custom Foot Orthotics.
- For all other insurances, payment is due at visit and a receipt is issued for you to submit for reimbursement.
